  1. Date and time, plus Author is pre-filled in for you.  This is the date / time that you are entering the log entry, not necessarily the time in which the incident occured.

  2. Choose an appropriate log type

    1. Discipline - the ‘major’ type of incidents.

    2. Observations - normally a ‘Late to class x times’ or ‘unprepared’ normally entered by the teachers

    Enter a description of what happened into the log entry text box. 
    1. Chromebook - usually used by teachers to indicate “forgot chromebook” for the day (admin / teachers)

    2. Attendance, counselor, grade change, health and parent contact – these are categories that can be used only on the admin portal.

  3. Title is a short title of the incident.

  4. Log Entry Text is the description or what happened in the incident. These can be internal notes, shorthand version or a mini book if needed.

  5. Select the appropriate values for.  These The next couple are for internal tracking, and if . If they don’t apply to the incident, then leave them blank.

    1. Consequence - optional for discipline means what is the consequence granted.  If left blank it’ll go into the ‘dispensation queue’given.

    2. Reason - why was the consequence given incident entered.

    3. Incident Type - this This is a generic category of the incidentcategory

    4. Incident Type category - not in use; leave blank.

    5. Category - if you need a more descriptive category you can enter it in here.  Most of the time this is blank.

    6. Admin Notes - these are notes only an admin can see.  Most times this is left blankFounded - was the event founded or unfounded.

    7. Victim - this is a type in field, where you can start to type either the first or last name of the student (within the school) and it’ll autopopulate a list of student names.

    8. Incident Date - The day in which the incident occured. It can be different from the date and time of the incident and where it occurredentry (at the top).

    9. Incident Context - a context General location of where the incident occurred (during school hours, during a school event, etc.)occured.

    10. Incident Location and Loc Detail - If a location isn’t specified, there’s a box for you to enter in where the incident occurred.

    11. Report Name - Name of reporter (or person who saw the incident)

    12. Reporter Role - The role of the reporter

    13. Victim Type - What was the type of the victim

    14. Felony Flag - was the incident a felony?

    15. LIkely Injury - was there a likely injury?  (local district defined)

    16. School Rules Violation - was there any schools rules violated?

    17. Police Involved - Was there any police involvement?

    18. Hearing Officer - was there a hearing officer involved?

    19. Gang Related - was the incident related to gang involvement?

    20. Hate Crime - was the incident a hate crime?

    21. Alcohol Related - did the incident involve alcohol?

    22. Drug Related - did the incident involve - Another general location of where an incident occured

    23. Police Involved - Was the police invovled with the incident?

      1. If no, then skip to item X.

      2. If yes, then skip to contiune on.

    24. Victim Type - (hidden if police involved = No) - another field to describe the victim

    25. Felony Flag - (hidden if police involved = No) - Is this incident considered a felony?

    26. Likely Injury Flag - (hidden if police involved = No) - was the result of the incident likely to have an injury occured?

    27. School Rules Violation - (hidden if police involved = No) - was the incident a result of a school rule violation?

    28. Hearing Officer - (hidden if police involved = No) - was a hearing officer involved with the incident?

    29. Gang Related - (hidden if police involved = No) - was the incident gang related?

    30. Hate Crime - (hidden if police involved = No) - was the incident involved with any hate crimes?

    31. Alcohol Related - (hidden if police involved = No) - did the incident have any alcohol involved?

    32. Drug Related - (hidden if police involved = No) = was the incident involving drugs?

    37. Money Loss Value - if there is any monetary loss (IE: stealing of cash or something of that nature)

    38. Action Date - this is the date that the action should or will occur

    39. Action Taken - what was the action taken as a result of the incident

    40. Action Taken Detail - can’t choose anything, leave blank.

    41. Action Taken End - enter the end date that the action should or will end

    42. Duration (Assigned) - enter the duration of the action (IE: .5 of a day, 2 days, etc.)  If its blank, it’ll go into the ‘scheduling’ bucket

    43. Duration (Actual) - enter in the actual duration of the action (IE: .25 of a day, 1 day, etc.).  There are times when the assigned and actual are the same, that’s fine.  Just copy it in.

    44. Duration Change Source - why or what was the reason for the change between assigned and actual durations?

    45. Duration Notes - notes for during the duration if needed

    46. Consequence Notes (all the way at the bottom) - only used at JH

    47. Consequence Schedule (all the way at the bottom) - only used at JH

    48. Student Notified - Check this is you spoke with the student about the incident / consequence. If left blank it’ll go into the student notification’ bucket

    49. Parent Notified - Check this if you spoke with the parent about the incident / consequence.  Doesn’t email the parents or anything*  If left unchecked it’ll go into the ‘parent notification’ bucket.
      *Note: The teacher version DOES email the parent if its checked.  It's a school setting to allow emails from teachers -> parents, and will require some additionally templating of the emails.(hidden if police involved = No) - if the incident involved theft, approximately how much money of the items and/or dollar amount was loss?

    50. Student Notified - Was the student talked to about the incident? (If not checked, then it drops the event into the “student notification” bucket so that the administration can talk to the student about the event.

    51. Parent Notified - Was the parents notified? (Note: admin portal doesn’t email parents. However teacher portal does email the parents). {From teachers' perspective, if unchecked it goes into the “parent notification” bucket so that the administration can talk to the parents about the event]

Incident Type Code ISBE Definitions

Alcohol 

    1. Alcohol - The violation of laws or ordinances prohibiting the manufacture, sale, purchase, transportation, possession, or use of intoxicating alcoholic beverages or substances represented as alcohol. This would include being intoxicated at school, school-sponsored events, and on school sponsored transportation, or substances represented as alcohol. 

    2. Violence with physical injury

    1. - While on school grounds or under the supervision of school authorities, any conduct that involves the use, attempted use, or threatened use of force against the person or property of another, or any other offense that is a felony and that by, its nature, involves a substantial risk that physical force against the person or property of another may be used in the course of committing the offense, with or without a weapon, that results in injury requiring professional medical attention, e.g., stab or bullet wound, concussion, fractured or broken bone, or cut requiring stitches.
