How to Create a New Form

From the Forms Website

  1. Create a new form by going to https://forms.google.com (must be logged in to your Minooka 201 Google Account) 
  2. Select from the different choices to create a new form:
     

Blank: A new blank form to start from scratch

Template Form: A blank form with Minooka 201 Branding

(Your most recently used Forms will appear in this header - it won’t look the same for everyone)

By clicking on ‘Template Gallery’ you can see different types of templates that users have made and shared including Worksheets, Surveys, and more. Templates such as ‘Blank Quiz’ and ‘Assessment’ use features where you can assign point values to specific questions and responses. 

From Google Drive

  1. Navigate to Https://drive.google.com 
  2. In the top left- click New
  3. Select More - then Google Forms

Types of Questions

Typing an Answer

  1. Short Answer
    1. People can write answers in a few words
    2. You can set rules that people have to follow (Data Validation)
  2. Paragraph
    1. People can write long answers over one or more paragraphs
    2. You can set rules that people have to follow (Data Validation)

Choose From a List

  1. Multiple Choice
    1. People can choose between a set of options.
    2. People can choose one option.
    3. You can include ‘Other’ as an option, and people can type a short answer. 
    4. Based on their answers, you can send them to a certain section of the form. (Form Logic)
  2. Checkbox
    1. People can choose between a set of options
    2. People can choose one option

    5. You can set rules that people need to follow (Data Validation)
  3. Dropdown
    1. People can choose between a set of options

Upload Files

  1. Uploaded files will be stored within the Form Owner’s Google Drive
  2. You can specify which file types people can upload
  3. To fill out this question, respondents MUST be signed in to a Google Account

Choose from a Grid

  1. Linear Scale
    1. People can rate your question on a scale.
    2. Your scale can start at 0 or 1.
    3. Your scale can end on a whole number from 2 to 10.
    4. You can set a label for each end of the scale
  2. Multiple Choice Grid
    1. You can create a grid where people can select one answer per row.
    2. You can limit people to one choice per column
    3. You can shuffle the row order, if needed 
  3. Checkbox Grid
    1. You can create a grid where people can select one or more answers per row.
    2. You can limit people to one choice per column
    3. You can shuffle the row order, if needed

Form Settings

Click on the Gear icon in the upper right hand corner to access the settings menu

General Tab

  1. Check the ‘Require Email Address’ box if you want to force users to include their email address
    1. Checking the ‘Respond Receipts’ box under that will send users a response when their form has been submitted successfully
  2. When sending forms that parents will need to fill out, make sure the ‘Restrict users to Minooka 201’ is NOT checked
  3. Limit to 1 response if needed
  4. The bottom two boxes give users the ability to see data collected from the form, or change their answers, if needed.

Presentation Tab

  1. Show Progress bar - will show users how many questions they have answered, and how many are left to go
  2. Shuffle Question order - helpful if using this as a quiz to help deter students sharing quiz answers
  3. Show Link to submit another response - useful if parents would need to submit forms for each student. If the ‘Limit user to 1 response is checked’ - you want to make sure this setting is NOT checked. 
  4. Confirmation Message: What a user will see if receipts are on

  1. The ‘Make this form a quiz’ toggle must be on for the subsequent settings to be available
  2. Locked Mode on Chromebook - only use this if you know that every student will be taking the quiz on a Chromebook - not recommended for eLearning
  3. Release Grade: Release grades either immediately to the students, or wait to release at your discretion
  4. Respondent Can See: Check the boxes according to what you’d like the student to be able to see

**ALWAYS CLICK SAVE AFTER MAKING CHANGES ON ANY OF THE SETTINGS MENUS**

Other Settings

Click on the palette to change things like color, font, or header options on the Form.

Sharing and Sending the Form

Once you are finished with the form, you should Preview it before sending, by clicking on the Eye Icon in the settings bar. This will show you exactly what the form will look like for a user prior to sending it out.

When you are ready to send the form out - click Send

Using the Email Tab you can send out the Form via email to specific recipients. If using this method, the ‘Include Form in Email’ box MUST be checked. Use this method if you are only sending to one or two people

Use the Links tab to copy and then paste a link to the Form in some other method of communication. (Class Pages, Google Classroom, etc.) It is helpful to check the ‘Shorten URL’ box so the URL isn’t as long and complicated. Click the Copy button to copy the URL to your clipboard - then paste the link into your outgoing communication.

Viewing Responses

Once you have sent your form out - and have gotten responses back - the Responses tab will show how many responses you have gotten - and by clicking on that tab you can see the results

  1. The Summary tab charts the results of the questions
  2. The Question tab shows results per question
  3. The Individual tab shows one individual’s responses to all questions on the Form
  4. Toggle the Accepting Responses button off when you are ready to turn the Form off. 

By clicking on the Google Sheets icon, you can view the results as a Sheet, where you can organize, sort, and review data.
