How to Create a New Form
From the Forms Website
Create a new form by going to https://forms.google.com (must be logged in to your Minooka 201 Google Account)Â
Select from the different choices to create a new form:

Blank: A new blank form to start from scratch
Template Form: A blank form with Minooka 201 Branding
(Your most recently used Forms will appear in this header - it won’t look the same for everyone)
By clicking on ‘Template Gallery’ you can see different types of templates that users have made and shared including Worksheets, Surveys, and more. Templates such as ‘Blank Quiz’ and ‘Assessment’ use features where you can assign point values to specific questions and responses.Â
From Google Drive
Navigate to Https://drive.google.comÂ
In the top left- click New

Short Answer
People can write answers in a few words

Choose From a List
Multiple Choice

Upload Files
Uploaded files will be stored within the Form Owner’s Google Drive

Choose from a Grid
Linear Scale
People can rate your question on a scale.
Your scale can start at 0 or 1.
Your scale can end on a whole number from 2 to 10.
You can set a label for each end of the scale
Rating
User can give a rating of 1-up to 10
Can also select a shape (for example you can use stars and give the question 5 stars)
Multiple Choice Grid
You can create a grid where people can select one answer per row.
You can limit people to one choice per column
You can shuffle the row order, if neededÂ
Checkbox Grid
You can create a grid where people can select one or more answers per row.
You can limit people to one choice per column

Click on Settings in the center of the page to access the settings menu

Sharing and Sending the Form
Once you are finished with the form, you should Preview it before sending, by clicking on the Eye Icon in the settings bar. This will show you exactly what the form will look like for a user prior to sending it out.

When you are ready to send the form out - click Send

Using the Email Tab you can send out the Form via email to specific recipients. If using this method, the ‘Include Form in Email’ box MUST be checked. Use this method if you are only sending to one or two people
Use the Links tab to copy and then paste a link to the Form in some other method of communication. (Class Pages, Google Classroom, etc.) It is helpful to check the ‘Shorten URL’ box so the URL isn’t as long and complicated. Click the Copy button to copy the URL to your clipboard - then paste the link into your outgoing communication.
Viewing Responses
Once you have sent your form out - and have gotten responses back - the Responses tab will show how many responses you have gotten - and by clicking on that tab you can see the results
The Summary tab charts the results of the questions
The Question tab shows results per question
The Individual tab shows one individual’s responses to all questions on the Form
Toggle the Accepting Responses button off when you are ready to turn the Form off.Â
By clicking on the Google Sheets icon, you can view the results as a Sheet, where you can organize, sort, and review data.
