      2. Entry Date = Date of starting @ HS for Math Class

      3. Entry Code = Entry Code @ HS (should be a 01 - transfer in from another district)

      4. Exit Date = Ending date @ HS for Math Class

      5. Grade Level = 8

      6. Under IL State block:

         1. Enter in Home RCDTS code = blank

         2. Enter in Serving School = Minooka HS RCDTS

         3. Enter in Service Provider = Minooka JH RCDTS

         4. Percent Date Attended / FTE = .11

2. Save.
